WHY:                     
Few topics are moreimportant for providers today than implementing programs and solutions to accelerate the transformation necessary to prosper in the new world of value-based care. CHA’s perspective on this topic is of particular interest since robust systems and models for population health management have not been standardized in pediatrics. Much of the effort nationwide in developing those models and methods has focused on adult populations and fails to take into account the unique developmental needs of children and their families.
