Director of Quality for MidState Health Center in New Hampshire is a nursingprofession pioneer in attaining CMS Triple Aim goals
 
AUSTIN, TX – May 7, 2014 – In honor of the National NursesWeek celebration this week, e-MDs, a leading provider ofelectronic health records (EHRs) and practice management(PM) solutions, today recognized PeggyRosen, BSN, MA, the director of quality at Mid-State HealthCenter, for her pioneering work in quality initiatives.


Mid-State Health Center is a Federally Qualified HealthCenter (FQHC) in Plymouth, NH, and also a partner in the NorthCountry Accountable Care Organization(ACO), headquartered in Littleton, NH.


Rosen is a nationally recognized leader in healthcare forher work in advancing patient care and population health. She leads efforts to optimize the performance of the Mid-State Health Center, creating a model organization that meets the goals of the Centers for Medicare and Medicaid’s (CMS) Triple Aim quality initiative.


“Peggy Rosen has been leading the charge for yearswith her pioneering work on quality initiatives such as patient care, population health and Patient-Centered Medical Homes (PCMH),” said David L. Winn, MD, FAAFP, Chairman of the Board and CEO at e-MDs. “Working with her highly innovative staff, Rosen helped Mid-State Health Center attain the goals of the Triple Aim before the term even became popular. e-MDs is delighted and privileged to have the opportunity to work with a true innovator in Peggy. Originally developed by the Institute for HealthcareImprovement (IHI), the goals of the Triple Aim program have been adopted by the CMS. The goals are:


·        Improving the patientexperience of care – including quality and satisfaction
·        Improving the healthof populations
·        Reducing the percapita cost of healthcare


“We value the power that e-MDs’ Electronic Health Record offersto help us meet our quality and Triple Aim initiatives,” said Rosen. “Without a responsive EHR, we would not be able to implement new quality measures quickly. When something new comes up, we don’t have to wait six months for the next update – we have found that the e-MDs EHR is flexible and robust enough to adapt to any new quality initiatives.”


Rosen, who has worked on quality issues with e-MDs for thepast seven years, said its EHR also enables Mid-State Health Center to translate data it needs to manage population health. Rosen works with the Mid-State team to leverage the functionality of the EHR for population health, coordinated care, and patient engagement.


“e-MDs continues to be a valued partner in Mid-State HealthCenter’s quality initiatives,” said Rosen. “We were one of the first practices in the country to implement e-MDs Patient Portal, and our patients appreciate having that virtual connection available at their convenience. It continues to be one of our key patient engagement initiatives today.”


Rosen was also the leader of Mid-State Health Center’sPatients Are Central to the Team (PACT) project. Its goal was to engage patients in playing a central role in their healthcare team by promoting increasing patient and care-giver involvement in goal-setting and care planning.


PACT has allowed Mid-State Health Center, a Level 3 PCMH, toestablish a corporate culture of patient-centered care, improve patient understanding of the concept of patient-centered care, more fully engage patients as partners in their care and ultimately improve the overall patient experience. The scope and reach of Mid-State’s Patient Expert Advisory Team (PEAT), which was formed during the PACT project, has continued to grow over the past three years.


Rosen currently leads Mid-State’s Anywhere Care pilotprogram. Patients at locations outside Mid-State Health Center facilities will be able to access Anywhere Care’s secure videoconferencing on digital devices to meet with primary care physicians at Mid-State Health.
