Netsmart Partners with The Arc Erie County and The Arc of Monroe County to Enhance Care Coordination and Evidence-Based Care Programs
 
OVERLAND PARK, Kan., Dec. 14, 2021 — Two member agencies of The Arc New York collaborative have partnered with Netsmart to enhance care coordination and delivery in a range of settings utilizing the Netsmart CareFabric[sup]®[/sup] platform. The Arc Erie County and The Arc of Monroe County will use the comprehensive suite of solutions to seamlessly connect data from individual care plans and optimize documentation and clinical workflows to better serve their evidence-based care programs.

“As our agencies have grown, we found our disparate systems were hindering our staff workflows, complicating documentation and were not providing actionable data. When the software does not move into the future with you, it limits the potential for the agency,” said The Arc Erie County CEO Doug DiGesare. “By partnering with Netsmart we will be able to take a more holistic approach to our agency’s system with an integrated, robust platform that can deliver quality metrics about an individual’s life to help provide better services and to ensure we have business analytics and data at our fingertips to make good business decisions.”

Combined, the two Arc agencies support more than 3,000 children and adults with intellectual and developmental disabilities (I/DD), autism spectrum disorder (ASD) and other learning disabilities and their families. The agencies will utilize CareFabric solutions that are built to increase the efficiency of the organizations. The Netsmart platform will deliver tools to improve care coordination across the continuum, solutions for direct service professionals to document care regardless of where or how that care is delivered along with tools to manage a diverse workforce effectively and efficiently.

The highly configurable technology is architected so providers have the tools to deliver person-centered support to individuals with dignity, respect and involvement in the decision-making about one’s health and personal goals. The Netsmart CareFabric platform is designed to fully empower agencies committed to the four principles of person-centered care: dignity, coordinated care, personalized care and assisting an individual to live independently.

Both agencies will leverage the mobile capabilities of the integrated platform, as well as comprehensive case management, billing and financial features.

In addition to capabilities such as interoperability, the two Arc agencies will also leverage key performance indicator (KPI) dashboards for clinical, operational and financial analytics. The Arcs will have the ability to create user-defined views across the organizations and filter aggregated data by program, service type, client access and much more.

“We were looking for a true partnership that could grow with us and believe that having the same software in a number of chapters will provide a lot of benefits as we look to the future as our organizations evolve and change,” said DiGesare. “Along with other Arc affiliates, we will be launching an Arc Advisory Council with Netsmart, a strategic initiative to unite all The Arc chapters utilizing the Netsmart platform and work more collectively on programs and projects.”

By using common platforms for solutions, the chapters will be able to collaborate on regional and statewide data, ultimately sharing better information. The advisory council will ensure technology, market demands and strategy are aligned. The council will meet regularly to facilitate discussion on current and future state and federal policy initiates and community advocacy.

The Arc of Monroe
The Arc of Monroe has been providing a variety of programs and services for over 1,700 people with intellectual and/or developmental disabilities and their families in the Rochester community for over 65 years. The Arc works to enhance the quality of life and self-esteem of people in our programs, providing them with meaningful social development, supported employment, residential community living and enrichment opportunities. The Arc is funded in part by the NYS Office for People with Developmental Disabilities. The Arc also receives funding from Adult Career and Continuing Education Services-Vocational Rehabilitation (ACCES-VR).

The Arc of Monroe has 40 locations throughout Monroe County that assist people with intellectual and/or other developmental disabilities that include Autism, Down Syndrome, Cerebral Palsy, Asperger Syndrome, and Pervasive Developmental Disorders. The Arc offers a wide array of services for people with intellectual and/or other developmental disabilities including nursing, clinical (PT, OT, SLP, and social work), living opportunities in our group homes and residences, volunteer opportunities in the community, unique and creative individualized day services programs, employment training at our ArcWorks Integrated Business, and employment training and placement through our Job Path Program.
