Shasta County Advances Adoption of the Netsmart Platform to Support CalAIM and Achieve Whole-Person Care for California Residents
 
OVERLAND PARK, Kan., Oct. 12, 2022 —  Netsmart and the Shasta County Health & Human Services Agency today announced an extended partnership leveraging advanced solutions and services of the Netsmart CareFabric® platform to provide whole-person care to California residents. Through this partnership, Shasta County will utilize a full suite of solutions from Netsmart to integrate with the county’s and consumers’ other care providers and establish shared clinical workflows that support the California Advancing and Innovating Medi-Cal (CalAIM) program. 

Shasta County will utilize several components of the CareFabric platform to support CalAIM program initiatives including, data analytics, consumer engagement, telehealth and care coordination tools. The power of these solutions will drive clinical and consumer engagement; creating provider efficiencies in delivering integrated virtual care. Additionally, the agency will leverage the industry-leading electronic health record (EHR) myAvatar™ designed to simplify the coordination of complex behavioral health services in community-based, residential and inpatient programs across the county’s geographic footprint.  

CalAIM promises to strengthen Medi-Cal by offering individuals a more person-centered approach to providing and receiving care in all aspects of life. Based on two waivers, Medi-Cal 2020 Section 1115 demonstration and CalAIM Section 1915(b), along with several state plan amendments, CalAIM initiatives including Enhanced Care Management (ECM) and Community Supports (CS) will improve care coordination, access to services and the overall health of more than 14 million California Medi-Cal enrollees.

The agencies using the Netsmart platform will benefit from next-generation technology that strengthens enhanced care management and community support services. To improve these services, Shasta County will utilize CareConnect™, an integration engine to advance interoperability by using direct secure messaging (DSM) and health information exchanges (HIEs) to drive care transitions and data exchange across county providers. Tracking key performance indicators through dashboards will identify gaps in care and provide data insight on their client population to help providers achieve top clinical outcomes for the services they deliver at the lowest cost. 

Since 2017, seven Netsmart California County clients have used the CareConnect network to facilitate 1 billion care coordination transactions between their behavioral health populations and 15 of their health system partners. At the current pace of adoption, there will be more than 500 million transactions in 2022 alone.
