HealthSouth’s Denise Barbera, BSN, MA, Chief Nursing Officer, and Kerrin Chrupcala, Compensation Liaison, will co-present a standardized approach to handoffs and transfers communication to assure a patient’s smooth, safe and timely transition to the next care setting. This approach promotes better communication of the patient’s status, and care among providers and others involved in the patient’s care. The webinar also covers the information contributing to the handoff’s objective for the patient’s safe and timely transfer─current information such as the patient’s care, treatment and services, medications, present condition, and recent or anticipated changes.
