Zynx Health’s Grant Campbell and Baylor Scott & White Health’s Nurse Professional Development Specialists to Speak at SINI 2015
At the University of Maryland School of Nursing’s 2015 Summer Institute in Nursing Informatics (SINI) conference in Baltimore, Maryland, Grant Campbell, M.S.N., R.N., senior director of Nursing Strategy and Informatics at Zynx Health™, will present “Utilizing A Mobile Care Coordination Platform to Improve Transitions of Care.”
Baylor Scott & White Health’s D’Ann Orr, M.S.N., R.N., C.H.T.S.-C.P, C.C.R.P., and Laurie Herzer, B.S.N., R.N., M.P.H., O.C.N.[sup]®[/sup], both nurse professional development specialists III, will co-deliver a poster presentation on “Building A Better Care Plan.”

PRESENTATION TOPICS:
Campbell will describe how one accountable care organization (ACO) in Northern California deployed a shared mobile care coordination communication platform powered by ZynxCarebook™ to improve its ability to support transition needs of patients in moving from acute care to post-acute settings. The ACO realized a 40 percent increase in post-discharge referrals to case management, 30 percent increase in post-discharge appointments kept, 39 percent increase in medication discrepancy, and a 10.2 percent readmission rate (compared to Medicare average of 21 percent).
Orr and Herzer will co-present Baylor Scott & White Health’s plan to update and simplify its electronic care plan templates to be more concise, meaningful and evidence-based via the ZynxCare[sup]®[/sup] solution. The 18-month process at the Temple, Texas-based healthcare organization resulted in a reduction of redundant problems and interventions, and minimized verbiage and decreased mouse clicks that improved user satisfaction and compliance.
