Ensocare and Henry Mayo Partner to Streamline Patient Discharge
By automating the care transition process with the help of Ensocare Transition and Ensocare Choice, Henry Mayo Newhall Hospital will reduce the time patients spend in acute care while empowering patients and their families in their post-acute care decisions.

OMAHA, Neb. – September 23, 2020 - A Valencia, California-based not-for-profit community hospital has made a move that emphatically demonstrates its goal of creating an ideal experience for patients, employees and the community.

Henry Mayo Newhall Hospital has entered into an agreement with Ensocare to make the Transition and Choice software solutions an integral part of the patient discharge experience. By giving their case management team access to automated referral solutions that have been shown to reduce Length of Stay, minimize avoidable days and cut down on clerical hours spent focused on manual labor, Henry Mayo will not only empower its team to create an unmatched healthcare experience, but they will also ensure patients who need post-acute care don’t have to stay in the hospital any longer than necessary.

Ensocare Transition enables Henry Mayo case managers to send Patient Health Information (PHI) to applicable post-acute care providers via secure digital messaging. In addition, the patient-facing Ensocare Choice platform lets discharge planners showcase all relevant post-acute care providers to patients and their families, documenting their choice directly within the Electronic Health Record (EHR) and ensuring they’ll be able to select the provider that’s right for them.
