Ensocare and Henry Mayo Partner to Streamline Patient Discharge
By automating the care transition process with the help of Ensocare Transition and Ensocare Choice, Henry Mayo Newhall Hospital will reduce the time patients spend in acute care while empowering patients and their families in their post-acute care decisions.

OMAHA, Neb. – September 23, 2020 - A Valencia, California-based not-for-profit community hospital has made a move that emphatically demonstrates its goal of creating an ideal experience for patients, employees and the community.

Henry Mayo Newhall Hospital has entered into an agreement with Ensocare to make the Transition and Choice software solutions an integral part of the patient discharge experience. By giving their case management team access to automated referral solutions that have been shown to reduce Length of Stay, minimize avoidable days and cut down on clerical hours spent focused on manual labor, Henry Mayo will not only empower its team to create an unmatched healthcare experience, but they will also ensure patients who need post-acute care don’t have to stay in the hospital any longer than necessary.

“This is a gamechanger for our team,” said Jo Kaplan, Director of Case Management at Henry Mayo. “We provide a level of patient care that you can’t find anywhere else, and part of that experience is making sure our patients get precisely the type of treatment they need without requiring them to stay in the hospital any longer than necessary.

“With Ensocare’s automated software, we’re able to give patients more choices than ever before in where they’d like to receive post-acute care. And when the patient is ready, Ensocare will allow us to discharge that individual quickly and safely, so that they can continue on their prescribed care journey in the most efficient way possible.”

Ensocare Transition enables Henry Mayo case managers to send Patient Health Information (PHI) to applicable post-acute care providers via secure digital messaging. In addition, the patient-facing Ensocare Choice platform lets discharge planners showcase all relevant post-acute care providers to patients and their families, documenting their choice directly within the Electronic Health Record (EHR) and ensuring they’ll be able to select the provider that’s right for them.

“I’m thrilled Ensocare will be able to make such a substantial difference in the lives of Henry Mayo’s patients,” said Luis Castillo, president and CEO of Ensocare. “These two solutions, working in connection with one another, raise the bar for optimizing transitions of care. In addition to ensuring compliance with Medicare’s Conditions of Participation, the care team at Henry Mayo will be able to spend more time doing what they’re great at: working at top of license while providing an unmatched patient experience.”
