Jeanette Ball, BSN, R.N., PCMHCCE, Delivery Manager, Advisory Services, CTG, will present “Technology as a FirmFoundation for Successful PCMH Implementation” at the MGMA 2015 AnnualConferencein Nashville, Tennessee. Named among the first100 National Committee for Quality Assurance (NCQA) patient-centered medical home (PCMH) content expertsin the nation, Ball is a healthcare executive leader with more than 28 years of clinical nursing and operational experience. She has led three community-wide Quality and PCMH projects impacting more than 300 practices and 900 providers, and has personally assisted dozens of practices in their attainment of NCQA PCMH recognition.

PRESENTATION TOPIC:
The patient-centered medical home (PCMH)model has received growing attention as communities improve the quality of patient populations by delivering high-quality, cost-effective accessible care. The PCMH model transforms how care is delivered by strengthening the physician-patient relationship through a model of coordinated, patient-partnered healthcare and ongoing quality measures. This is especially important with increasing focus on the Delivery System Reform Incentive Payment (DSRIP) program affecting cost delivery for underserved vulnerable populations in many states.
 
Using technology is the enabler that candrive significant change if coordinated and in synch with clinical and quality programs. PCMH offers a guided path toward understanding and affecting an organization’s population health. Ball will explore how PCMH was used to improve diabetic health outcomes in a community-wide PCMH initiative across western New York, covering an eight-county area and more than 200 practices. She will also discuss how PCMH can be used to effectively prepare practices for population management and continuous quality-driven programs.
