Netsmart Partners with Bernalillo County to Increase Successful Post-Incarceration Transitions Back into Society
 
OVERLAND PARK, Kan., Aug. 11, 2020 — Netsmart announced today a partnership with New Mexico’s largest county, Bernalillo County, to implement CareManager as their population health management and care coordination platform to improve data sharing and analytics capabilities, with the goal of improving the management of individuals re-transitioning back into the community following incarceration. This partnership will leverage the platform to deliver coordinated care and seamless information sharing with the Comprehensive Assessment and Resiliency through Excellence (CARE) campus, Metropolitan Detention Center (MDC) and the award winning Resource Re-entry Center (RRC).  

Bernalillo County will utilize the Netsmart CareManager solution to share client information between jail providers, the CARE campus and the RRC to showcase analytics on program success for the county and legislation. The CARE Campus, operated by the Department of Behavioral Health Services, will use the client management and data integration system to better serve clients in each of the four main programs it operates: the Public Inebriate Intervention Program, Social Model Detox, the Crisis Stabilization Unit and its Supportive Aftercare Program. 

“The partnership with Netsmart will allow us to have information about the individuals we serve at our fingertips, including jail assessments, information on Social Determinants of Health and other vital data about the individual as they transition back into the community” said Evan Gonzales, Public Information Officer of Behavioral Health Services, Bernalillo County. “With over 11,000 individuals served each year, having information and better communication across systems of care is vital to ensure the successful transition into the community.”

The Bernalillo County Department of Behavioral Health Services was recently recognized with an achievement award from the National Association of Counties (NACo). The NACo awards honor innovative, effective county government programs that strengthen services for residents. Bernalillo County earned best in category for the Supportive Aftercare Program for their community reintegration efforts. The data integration system provided by Netsmart will further enhance the CARE campus’s services ensuring its standing as a nationally renowned program.

In addition to the care coordination platform, Netsmart will support the technology implementation with data collection and integration for the RRC program. The RRC is a program developed out of the Behavioral Health Initiative, a program that provides reintegration support to releasing inmates from the Metropolitan Detention Center by offering access to the services and support they may need to stabilize, stay healthy, regain self-sufficiency and break the cycle of recidivism.

“We want to provide our clients with support and services to get them back on their feet as they transition into society, so together we break the cycle of recidivism,” said Margarita Chavez- Sanchez, Director of Behavioral Health Services, Bernalillo County. “Our utilization of services at the RRC has tripled by creating a safe space where our staff and case managers can offer our clients something as simple as a hot cup of coffee and then sit down to look at what services we can help them with as they transition into the community.” 

The Bernalillo County RRC Inaugural Year Review shows the RRC sees anywhere from 50-70 individuals released from jail each day.

The collaboration between Netsmart and Bernalillo County to leverage the population health management and care coordination platform is an innovative technology enablement initiative that will improve data sharing, provide capacity to analyze community behavioral health needs and connect clients to the services they need.
