WHY:                     
Few topics are moreimportant for providers today than implementing programs and solutions to accelerate the transformation necessary to prosper in the new world of value-based care. CHA’s perspective on this topic is of particular interest since robust systems and models for population health management have not been standardized in pediatrics.
