PRESENTATION
TOPIC:
Healthcare organizations have historicallydepended upon an unreliable patient handoff and discharge process, placing patient care quality and safety at risk while increasing costly readmissions. Calling upon his 25 years of experience as a staff nurse, educator and leader in critical care settings, Campbell will discuss how organizations use evidence-based content and mobile technology solutions to improve this process.
 
Using real-world examples, Campbell will: show nurses how they can employevidence-based content to reengineer pre- and post-discharge care processes to deliver better outcomes; demonstrate how a mobile care coordination tool can support effective real-time communication among a vast array of on-the-go caregivers; and recognize how this improved care communication can lead to improved results, including reductions in readmissions and length of stay.
